How can I create new Question and Answers in the Lead Form?

Note: New questions and answers are added on the same level underneath the selected item

New question

1. Select the item you want to add a new questions underneath
   

1. Click on the 'New Question' button on the left
   
2. Enter 'Caption' and 'Description' for the new question
   
3. Select, if you want to offer 'Single' or 'Multiple Answer'
   
4. Click 'Save & Close' to create the new question
   

1. The new question is created. Now you have to define the answers.

1. Click on the 'New Answer' button to the left
   
2. Enter 'Caption' and 'Description' for the new question
   
3. Select, if you want to allow 'No Note', 'Can add note' or 'A Note must be added'
   
4. Click 'Save & Close' to create the new answer
   

1. The new answer is created. Add further if required.
